To receive services, please follow the steps below:
- Complete the Client Intake Form and submit it via email (intake@sheltercare.org), drop-off at our 499 location, or via phone (541.505.5188) with support from our office manager.
- Our office manager will schedule you for a brief, 15-30-minute Intake Screening with one of our Qualified Mental Health Associates (QMHAs). The Intake Screening can be completed via phone, telehealth, or in-office. During this meeting, we will determine if we are the best fit for your treatment needs. We will review your presenting concerns, treatment history, and communicate important information related to our supports and policies. - BH Cancellation Policy
- Electronic Communication Agreement
- If we are not the right fit for your needs, we will assist you in identifying other options for support.
- If we find that we are a treatment match, you will be scheduled for a one-to-two-hour Comprehensive Mental Health Assessment with your assigned therapist. We have found that in-office assessments give us a deeper understanding of our clients’ experience and needs. If you would like a telehealth accommodation for this appointment, please let us know.
